1.0 SITE INDUCTION & REGISTRATION
1.1 All contractors' personnel are required to attend the site specific induction training that will be delivered by AKP staff and to comply with all of the requirements contained therein, together with all site rules published and amended by the AKP site management team.
1.2 For the purposes of safety, all persons on site must sign-in immediately on attending site and sign out when leaving site.
•2.0 RISK ASSESSMENTS & METHOD STATEMENTS
2.1 In accordance with the requirements of the Management of Health and Safety at Work Regulations 1999, suitable and sufficient risk assessments and method statements must be submitted for approval no later that 10 days prior to anticipated commencement date on site. Works must not be commenced unless these documents are available on site and have been communicated to those affected by the works. Evidence of communication will be required.
3.0 WORK AT HEIGHT
3.1 All works at height must be undertaken from suitably constructed working platforms, erected only by competent persons. Evidence of training for those constructing working platforms must be provided to the site manager prior to commencement. The contractor is responsible for all inspections of work platforms in accordance with the requirements of the Work at Height Regulations 2005.
3.2 Stepladders shall not normally be permitted unless the risk assessment for the works demonstrates that these are the most appropriate form of work equipment. This will normally occur only when the physical constraints of the site dictate and the works are of an isolated, short term and light duty nature. The use of stepladders on site is controlled by permit to work.
3.3 The sub-contractor will provide evidence of training for the erection, modification and dismantling of work at height equipment. This will include PASMA (or equivalent) training for mobile scaffolds and IPAF training for mobile elevated working platforms.
4.0 HEALTH & SAFETY ASSISTANCE
4.1 The contractor shall appoint one or more competent persons to assist him in complying with the requirements of health and safety legislation in accordance with the requirements of the Management of Health and Safety at Work Regulations 1999. The name of such persons appointed shall be provided to AKP on request.
•5.0 CONTRACTOR ASSESSMENT QUESTIONNAIRE
5.1 The contractor shall complete to the satisfaction of AKP, any assessment questionnaire issued in the interests of compliance with the Construction (Design and Management) Regulations 2007.
•6.0 COMPLIANCE WITH LEGISLATION
6.1 The contractor will undertake all works in compliance with the Health and Safety at Work etc. Act 1974 and all subordinate legislation, with particular emphasis on the Construction (Design and Management) Regulations 2007.
•7.0 PERMITS TO WORK
7.1 AKP Scotland operates a permit to work system for the following aspects of work on site: -
7.1.1 Work from Stepladders
7.1.2 Work on or access to Roofs
7.1.3 Confined Space Working
7.1.4 Hot Works (Works that generate temporary ignition sources)
7.1.5 Works local to electrical services
7.1.6 Excavations or breaking ground
Works involving any of these aspects must not be undertaken until an authorised permit to work has been issued by the AKP site management team.
8.0 SITE CLEANLINESS & GOOD ORDER
8.1 A high standard of good order and site cleanliness must be maintained on site at all times throughout the duration of the works. Waste must be removed progressively and must not be permitted to accumulate until the end of the works. In the event of any contractor failing to meet this requirement, AKP reserve the right to remove contractors waste and to make deductions from the contractors account accordingly.
9.0 SMOKING
•10.0 LIGHTING, SIGNS & BARRIERS
10.1 The contractor shall provide sufficient task lighting, signs, barriers, temporary fencing, boarding, screens, planked footways, guard rails and similar items as may be necessary for the works and to warn and protect other persons on site from dangers whilst carrying out the work.
11.0 WORK EQUIPMENT
11.2 The sub contractor shall provide evidence of training for specialist items of plant and equipment.
12.0 FIRE PRECAUTIONS
12.1 The contractor is to take all reasonable precautions to avoid the outbreak of fire particularly in work involving the use of naked flames or smoke generating equipment. Such work should be examined at short intervals immediately following its completion. The contractor will undertake his works in accordance with the current Code of Practice for Fire Prevention on Construction Sites published by the Fire Protection Association.
12.2 Fire escape routes are to be kept unobstructed at all times.
12.3 For work involving temporary sources of ignition (hot works), the contractor is to provide and maintain suitable portable fire extinguishers which are to be readily available.
12.4 Doors marked 'fire-door -keep shut' must not be wedged or otherwise fastened in the open position under any circumstances.
12.6 The contractor shall follow the AKP procedures for action in case of fire, and ensure that his employees are aware of and fully comply with the AKP fire drill and reporting procedure.
13.0 TEMPORARY SITE HUTS AND COMPOUNDS
13.1 The contractor shall obtain written permission from AKP for the erection of temporary buildings or compounds, and shall discuss with AKP the siting and length of time they will be required.
13.2 AKP not accept liability for loss of the contractors site hut, contents, temporary buildings or storage areas, nor shall be responsible for the security patrolling of them.
13.3 Irrespective of any shared welfare arrangements provided by AKP, the contractor remains responsible for ensuring the provision of welfare facilities to his employees in accordance with the Construction (Design and Management) Regulations 2007.
14.0 REPORTING OF INJURIES, DISEASES AND DANGEROUS OCCURRENCES
14.1 The contractor shall report on all accidents to his employees (and others who suffer injury/ill health as a result of his activities) and dangerous occurrences on AKP sites to the AKP Site Manager.
14.2 For RIDDOR notifiable injuries or dangerous occurrences, the contractor will comply with the reporting requirements of legislation and thereafter provide AKP will all relevant information in accordance with the Construction (Design and Management) Regulations 2007.
15.0 PROVISION OF FIRST AID FACILITIES
15.1 Irrespective of any shared first aid provision made available on site, the contractor remains responsible for discharging his statutory duties for the provision of first aid facilities under the Health and Safety (First Aid) Regulations 1981.
16.0 PERSONAL PROTECTIVE EQUIPMENT
16.1 The contractor shall provide adequate personal protective equipment for all his employees and ensure his sub-contractor employees also wear any necessary personal protective equipment whilst working on any AKP site.
16.2 Minimum PPE requirements for AKP sites comprises safety helmet, safety footwear with ankle support and protective mid-sole, safety spectacles and gloves. Other PPE requirements shall be determined from the risk assessment undertaken for the activity.
•17.0 ASBESTOS
17.1 The contractor must review the asbestos register for the site and ensure his work methods ensure that known asbestos is not disturbed.
17.2 No contractor must undertake the removal of asbestos containing materials, except licensed contractors who are appointed and approved by AKP for that purpose.
17.3 In the event that asbestos is suspected or disturbed, the contractors' employee must immediately vacate the area and report the incident to the AKP Site Manager.
18.0 SMOKE DUST AND NOISE
18.1 The contractor must take all necessary precautions to prevent nuisance from smoke, dust, rubbish and other causes (including noise from radios etc).
19.0 BREACH OF HEALTH AND SAFETY RULES
19.1 The contractor shall undertake to remove from site at once, any of his employees, or employees of any sub-contractor found contravening any Health and Safety rules, regulations or instructions.
20.0 GENERAL CONDUCT
20.1 The contractor must conduct its work in such a manner that it does not place itself, AKP employees or any other persons or property at risk.
20.2 The contractor shall at all times comply with all appropriate and relevant requirements of health, safety and environmental legislation and approved codes of practice.
21.0 HYGIENE & INFECTION CONTROL (WORK WITHIN HEALTHCARE ENVIRONMENTS)
21.1 For works within hospital environments, the contractor will undertake his works and take all necessary precautions in accordance with the guidance contained with Scottish Health Facilities Note 30 - Infection Control in the Built Environment.
21.2 The contractor will undertake to provide training to his employees in general infection control precautions implemented throughout the duration of the works.
